Appointments: Patients are seen by appointment only. Please contact our office at 305-754-6240 to make an appointment.
Changing or Canceling Appoinments : We recognize your time is valuable and every effort will be made to accommodate your preferred appointment time. In order to ensure availability to our patients we require 24 hours notice if you need to cancel an appointment.
If you do not provide 24 hours notice to cancel an appointment or do not keep a scheduled appointment you may be charged a "no show" fee.
New patient Information and Forms: We welcome you as a new patient to our practice and look forward to making your registration experience convenient. As a new patient to Miami Shores Neurology & Sleep Institute, we request the following information which will help us to conduct a through evaluation:
Please allot at least 60 minutes for your initial appointment
Arrive 20 minutes prior to your appointment to complete your registration
On the day of consultation please bring the following if applicable:
Completed - Patient Registration Packet (All new patients)
Prescription Refills: Please call the office at (305) 754-6240 for any prescription refills or questions regarding your prescription.
Insurance programs accepted by Miami Shores Neurology and Sleep Institute: Medicare and Commercial HMO and PPO insurance programs.
Financial Policies: The providers and staff of the Miami Shores Neurology and Sleep Institute are committed to helping you receive quality care and make your experience as convenient as possible. In order to acheive this goal we require our patients to follow our financial policies and procedures.

Checks are not accepted as a form of payment
For your convenience credit cards or cash are accepted as payment on accounts
Please notify our office of any insurance coverage changes before your appointment
Please be prepared to pay co-insurance and deductibles the day of your visit
Credit card payment processing is available for your convenience
Please keep your account current with our practice
All unpaid balances will incurr a 1.5% finance charge per month
Patients are responsible to obtain authorization from the PCP before their visit -(if required by your insurance carrier)
Services considered non-covered by your insurance plan are the responsibility of the patient or their representative
We will submit insurance claims on your behalf, however the balance of the patient account is the responsibility of the patient or their representative
There is a $20 charge for completion of forms (SSI disability, FMLA, etc.)
There is a nominal fee for medical record copies (based on record size)
